Methods of Registration

Online method

Please visit CSIS, select "User Registration - Service Provider" on the main menu. Follow the instructions to complete the on-line registration form.

Offline method

You may fill in the Registration Form for Service Provider (which can be downloaded from the CSP website (www.cervicalscreening.gov.hk). After completing the form, please send it back to CSP. by fax (2833 5445) or by mail to:

After your application has been approved, we shall inform you by mail.
