Online method

Online method You may register online in the CSIS. After filling in the online form, an Endorsement Form will be generated based on your inputted information. Please print out this from and have the form countersigned by your laboratory person-in-charge. Then, send it back by fax (2833 5445) or by mail to:

Offline method

Offline method You may download the Registration Form for Laboratory Professionals. Complete the form, countersign by your laboratory person-in-charge and then send it back by fax (2833 5445) or by mail to:

If you work in different laboratories, please complete separate registration form for each of them. If you change your working laboratory in the future, you need to inform us and fill in a new registration form for your new laboratory. After your application has been approved, we shall inform you by mail.
