 
Methods of Registration

Registration through health professional
First, choose your own service provider for cervical smear. If he/she
has registered with the Cervical Screening Programme (CSP), you will
be asked to complete a Registration
Form for Women. This registration form is also a consent form
indicating that you agree to let your personal information and smear
results be accessed and transferred between service providers and
laboratories for continuity of care. Your service provider will then
send your completed form to us. If you change your service provider
in the future, you will be asked to fill in the form again.

Direct registration
If your service provider has not joined our programme, you may register
by sending your application to us directly using one of the following
ways:

Online method
Please visit CSIS,
select "User Registration - General Public" on the main
menu. Follow the instructions to complete the on-line registration
form. After registration, you need to provide a copy of your HKID
card to us by fax (2833 5445) or by mail to:

Please download the Registration
Form for Women. Complete the form and send it back together
with a copy of your HKID card either by fax (2833 5445) or by

After your application has been approved, an account to store
your smear results will be created in the CSIS.
We shall inform you by mail.
