Submitting Smear Results

If you join our programme through your service provider, he/she will send your smear results to the CSIS. If you join our programme by yourself, you may send your smear results to us by fax (2833 5445) or by mail to:

Self-held Smear Record

You may download the Cervical Screening Record Card. Its purpose is for you to keep a record of your own smear results to facilitate continuity of care by health professionals. Please ask your service provider to write down your smear results and date of smear on this card. Please bring it along when you attend for cervical smears or when you consult a doctor for gynaecological symptoms.
